Anyone wanting fuller or more defined lips often faces the question: lip lift or lip filler? Both methods can alter the appearance of the lips, but they pursue different goals. While hyaluronic acid primarily creates additional volume and emphasises the lip contours, a surgical lip lift elevates the upper lip and shortens the distance to the nose. Which treatment is more suitable therefore depends on your individual facial proportions and the desired result. Why do lips sometimes appear too thin or lack volume?
How full and pronounced the Lips appearing, depends on individual anatomy, genetic predisposition, and age-related changes.
Frequently, several factors work together:
- Genetically thin lips
The shape, size and volume of the lips are largely determined by genetics. Some people therefore naturally have a narrow upper or lower lip, poorly defined contours or very little visible lip colour. - A large distance between the nose and upper lip
The philtrum refers to the area between the nose and upper lip. If this distance is comparatively large, the upper lip can curve outwards less. As a result, less vermilion border is often visible and the upper lip appears narrower. - An inverted upper lip
In some people, the upper lip rolls slightly inwards. As a result, less vermilion shows from the front, even though there may fundamentally be sufficient lip tissue. The upper lip therefore appears narrower and less defined. - Age-related volume loss
As we age, the body produces less collagen and hyaluronic acid. As a result, the lips gradually lose their fullness and elasticity. At the same time, the red part of the lips can shift slightly inwards, making the lips appear narrower and less defined. - Unbalanced proportions or asymmetries
For example, if the upper lip is significantly narrower than the lower lip or one side is less pronounced than the other, the shape of the lips can appear unbalanced. Even small differences between the halves of the face can influence the contour and overall impression of the lips.
Therefore, for choosing the appropriate treatment, it is crucial why the lips are perceived as too narrow or lacking in volume.
Treating thin lips: lip filler or lip lift?
The stated causes already show: not every thin-looking lip needs to be plumped up. The crucial factor is whether volume is actually lacking or whether it is primarily the position of the upper lip that influences the appearance. This results in different treatment options:
Lip filler with hyaluronic acid
We inject these directly into your lips. Fillers can create additional volume, accentuate the contours and correct slight asymmetries. The result is temporary and the dosage can be tailored individually.
Bullhorn lip lift
This is a surgical procedure in which we shorten the distance between the nose and the upper lip. This lifts the upper lip, making more of the red part of the lip visible. The volume of the lip itself is not increased by means of a filler.
Put simply, lip fillers primarily provide more volume, whereas a lip lift changes the position and visible shape of the upper lip. Which method is more suitable therefore depends on your individual proportions and the result you wish to achieve.
How does a lip augmentation with hyaluronic acid work?
During a lip filler treatment, we inject a hyaluronic acid-based filler into your lips or along the lip contour.
Depending on the amount used and the placement, the treatment can pursue different objectives:
- more volume in the top lip or bottom lip
- a more clearly defined lip line
- the accentuation of the Cupid's bow
- a correction of slight asymmetries
- more harmonious proportions between the upper and lower lip
Treatment is carried out without surgical intervention and can be dosed comparatively precisely. The result is visible immediately after the injection. However, slight swelling or small bruising can still affect the appearance in the first few days.
As the body breaks down the hyaluronic acid over time, the result is not permanent. How long it remains visible depends, among other things, on the product used, the amount injected and your individual metabolism. If the volume is to be maintained, the treatment can be repeated following an appropriate medical assessment.
What is important is: lip fillers can pad out and shape. However, they cannot shorten a greater distance between the nose and the upper lip. If the cause of a narrow-looking upper lip is primarily a long philtrum or little visible lip vermillion, a pure filler treatment therefore reaches its limits.

How does a surgical lip lift work?
A lip lift, also known as a lip lift or bullhorn lift, is a surgical procedure to elevate the upper lip. In this process, we remove a narrow strip of skin directly beneath the nose, thereby shortening the distance between the nose and the upper lip. The upper lip is pulled slightly upwards, so that more of the vermilion border is visible.
The bullhorn lip lift can be particularly useful if:
- the distance between the nose and the upper lip is comparatively large,
- the upper lip appears long or turned inwards,
- only a little lip colour is visible,
- a long-term change in the lip shape is desired.
The incision usually runs along the natural transition beneath the nose. This allows the subsequent scar to be placed as inconspicuously as possible. Nevertheless, a lip lift remains a surgical procedure for which an appropriate healing time must be planned.
Unlike a lip filler, no additional material is introduced into the lips. The Bullhorn lip surgery therefore does not primarily create more volume, but rather alters the proportions and the visible shape of the upper lip. The result is long-lasting, but unlike treatment with hyaluronic acid, it cannot be gradually broken down or easily reversed.

- With the bullhorn lip lift, we raise the upper lip and accentuate its shape more.
- Repeated filler treatments are not required for this effect.
Potential disadvantages of a lip lift
- It is a surgical procedure.
- The healing time is longer than after a filler treatment.
- A scar is forming below the nose.
- The result cannot simply be undone.
- As with any operation, there are risks such as secondary haemorrhage, infections or wound healing disorders.
Both treatments can involve side effects and risks. Following an injection, for example, swelling, bruising or temporary irregularities may occur. With a surgical lip lift, wound healing, scarring and general surgical risks, among other things, must be taken into account.
Who is a lip filler suitable for and who is a lip lift suitable for?
Which treatment is suitable depends primarily on the individual anatomy and the desired result. Not every naturally thin-looking lip automatically requires more volume.
A lip filler with hyaluronic acid can be particularly suitable if you:
- desire more volume in the upper or lower lip,
- want to define the lip contours more clearly,
- wanting to have slight asymmetries corrected,
- first prefer a temporary and easily controllable modification,
- wish to have no surgical procedure.
A lip lift may be more suitable if you:
- have a comparatively large distance between the nose and upper lip,
- want to make more of their lip colour visible,
- wanting to correct an inwardly turned or poorly defined upper lip,
- desire a long-term change in lip proportions,
- not achieving the desired result despite repeated filler treatments.
A thin-looking upper lip is not automatically due to a lack of volume. During the personal consultation, we therefore first check whether there is an actual loss of volume or whether the position of the upper lip and a greater distance to the nose determine the appearance. Only then is it possible to assess which treatment can achieve a harmonious result.
Can lip lifting and lip fillers be combined?
The bullhorn lip lift and treatment with hyaluronic acid are not fundamentally mutually exclusive. Since both methods produce different changes, a combination can be useful in some cases.
The lip lift elevates the upper lip and makes more of the lip vermilion visible. If there is also a desire for additional volume or more targeted contouring, we can additionally inject hyaluronic acid. A restrained dosage is particularly important here so that the lips look natural and match the rest of the facial features.
However, we decide individually whether a combination is advisable. Following a surgical lip lift, you should first wait for healing and assess the final result before planning additional filler treatment.
Lip augmentation with autologous fat as a further option
Besides hyaluronic acid and a lip lift, autologous fat can also be used to build up lip volume. For this purpose, we first harvest fatty tissue from a suitable part of your body, such as the abdomen or thighs. We then prepare it and inject it into your lips. This procedure is called lipofilling.
As the tissue used comes from the patient's own body, no synthetic filler is used. However, the procedure is more complex than lip augmentation with hyaluronic acid, as first a small Liposuction is required. In addition, part of the transferred adipose tissue is reabsorbed by the body. The final volume is therefore less precisely predictable.
The adipose tissue that remains after the integration phase can ensure a longer-lasting result. Lip augmentation using autologous fat is therefore particularly suitable for individuals who desire more lip volume with the body's own material and do not shy away from the additional effort of fat harvesting.
