Swollen legs: What illnesses could be the cause?

Swollen legs

If legs increase significantly in size, harmless water retention is often suspected at first. However, if the change persists or is accompanied by pain, tenderness, feelings of tension, or an increased tendency to bruise, an illness may be the cause. Completely different processes in the body can play a role here – such as disorders of the blood or lymph flow, diseases of internal organs, or changes in fatty tissue. Accordingly, the symptoms and treatment options also differ. Below, you can find out which clinical pictures can lie behind thick legs, how they can be distinguished, and which therapies are available.

What are swollen legs?

The term „thick legs“ initially merely describes a visible and/or perceptible increase in leg circumference. This can affect the entire legs or be particularly noticeable on the thighs, lower legs or ankles. In some people, the legs only swell during the course of the day, while in others the change is permanent.

How the symptoms manifest precisely can already provide initial clues regarding the potential background. For instance, it plays a role whether both legs are equally affected or if changes are visible on only one side. Equally relevant is whether the swelling is associated with, for example, pain, feelings of tension, or tenderness to pressure.

What conditions can be behind swollen legs?

For fluid to be properly distributed in the body and transported away from the tissue again, various systems must work together. These include, in particular, the blood circulation and the lymphatic system. The texture and distribution of fatty tissue also influence the shape and circumference of the legs.

If one of these systems gets out of balance or if there is a disease of the tissue, this can become apparent in the legs.

Medical conditions that can be associated with swollen legs include:

Vein conditions

A common cause of swollen legs is chronic venous insufficiency. If the venous valves no longer close completely, some of the blood can flow back down into the leg veins. This increases the pressure in the vessels, whereupon more fluid is released from the capillaries – the body's smallest blood vessels – into the surrounding tissue. If this cannot be adequately drained away by the lymphatic system, so-called oedemas develop, which are swellings caused by fluid retention.

These usually occur on the lower legs and ankles. A feeling of heaviness or tightness as well as varicose veins may also occur. If venous insufficiency persists for a longer period, additional skin changes can develop, such as brownish to red-blue discoloration, dry or irritated skin, and hardening of the tissue. In advanced cases, slow-healing wounds on the lower leg can also develop.

If such swelling appears suddenly and in only one leg, particularly accompanied by pain, a feeling of warmth, or redness, a deep vein thrombosis is also possible. As this is a medical emergency, please have such symptoms evaluated by a doctor immediately.

The treatment of venous diseases depends on the precise diagnosis and may include compression and regular exercise, among other things. In the case of pronounced varicose veins, procedures such as sclerotherapy, laser or radiofrequency therapy, as well as surgical interventions, may also be considered depending on the findings.

Diseases of the heart, kidneys or liver

Diseases of the internal organs can also lead to fluid retention in the leg tissue. In heart failure, the heart can no longer pump the blood forward effectively, causing it to back up in the venous circulation and increasing the pressure within the vessels. Oedema typically affects both feet and lower legs. Depending on the severity, shortness of breath or reduced physical endurance may also occur.

The fluid balance can also be disrupted in certain kidney diseases, for example when water and salts are no longer excreted sufficiently or increased amounts of protein are lost through the urine. In severe liver diseases, on the other hand, the production of important blood proteins can be impaired, causing fluid to pass more easily from the blood vessels into the tissue.

Further treatment then depends on the respective findings, for example through diuretics, an adjustment of heart medication, or the treatment of the underlying kidney or liver disease.

Lymphoedema

The lymphatic system transports excess tissue fluid, proteins and other substances away from the tissue and returns them to the bloodstream. If this drainage is disrupted, fluid increasingly accumulates in the tissue and the affected leg swells. A Lymphoedema can be congenital or develop, for example, after surgeries, injuries or damage to the lymphatic vessels.

Typically, the swelling starts in the foot or ankle and can subsequently spread up the lower leg to the thigh. Unlike with lipoedema, the foot and toes can also be affected. Initially, the tissue often feels soft or doughy, and the swelling can still subside by elevating the limb. In cases of longer-standing lymphoedema, the connective tissue can increasingly harden, meaning the changes become permanent.

The treatment aims to support lymphatic drainage and reduce swelling. The basis is so-called complex physical decongestive therapy. This includes in particular compression, exercise, manual lymphatic drainage, and careful skin care.

Lipoedema

At the Lipoedema is it a chronic condition in which the subcutaneous adipose tissue on the extremities increases significantly and usually symmetrically. Particularly affected are the hips, upper and lower legs, and sometimes also the Poor. Typically, the fat distribution does not match the rest of the body proportions, while the feet and hands are spared.

In addition to the visible increase in volume, Complaints such as pain on pressure and touch, a feeling of heaviness and a pronounced tendency to bruise are part of the clinical picture. It is precisely these symptoms that help to distinguish a lipoedema from merely a heavy fat distribution on the legs.

Treating heavy legs due to lipoedema: how can liposuction help?

Whilst conservative measures such as compression and exercise primarily aim to alleviate the symptoms of a lipoedema, liposuction can directly reduce the pathologically increased subcutaneous adipose tissue. Liposuction does not serve the purpose of weight loss, but rather the treatment of the body regions affected by the lipoedema.

The aims of liposuction in lipoedema are:

  • Reduction of pathologically increased adipose tissue
  • Reduction in pain and tenderness
  • Improvement of mobility and physical resilience
  • Reduction of the size of the affected body regions
  • long-term relief in everyday life

At our Clinic for Plastic and Aesthetic Surgery in Weert we specialise in Liposuction for lipoedema. The VASER method is used for this. First, we loosen the fatty tissue using ultrasound energy and then suction it out using fine cannulas. In doing so, we ensure that the procedure is as gentle as possible on the tissue and lymphatic vessels.

Depending on the severity of the lipoedema and the size of the affected areas, several treatment sessions may be necessary. After the procedure, you will need to wear compression garments for a period of time to support the healing process.

From which stage of lipoedema is liposuction advisable?

Depending on the classification used, lipoedema is divided into various stages. Stages I to IV primarily describe how noticeably the fatty tissue and body shape have changed. However, they do not provide a reliable indication of how severe the symptoms are. For instance, sufferers with a less visible presentation can already experience considerable pain, whereas the symptoms in a more pronounced case can vary entirely from person to person.

Whether liposuction is advisable is therefore not decided solely on the basis of the stage. Rather, the decisive factors are the actual symptoms and the individual level of suffering. Surgical treatment may be an option if pain persists despite conservative therapy or if the lipoedema leads to mobility restrictions and further physical complaints. Potential concomitant diseases and body weight are also factored into the assessment.

Generally, liposuction can therefore be a treatment option regardless of the stage. Since October 2025, liposuction for lipoedema can be a benefit covered by statutory health insurance under certain conditions, regardless of the stage of the disease. The basis for this is a decision by the Federal Joint Committee (G-BA).

One too Treatment in another EU country, for example here in the Netherlands, can in principle be reimbursable for those with statutory health insurance from Germany. The costs are generally reimbursed up to a maximum of the amount that would be reimbursable for a corresponding treatment in Germany. You should therefore definitely clarify whether and to what extent your health insurance fund will cover the liposuction before undergoing the treatment. For hospital services in other EU countries, according to Section 13 (5) of the Social Security Code V (SGB V) prior authorisation from the health insurance fund is required.
